Job Duties and Responsibilities:
- Provide nursing services, within defined scope of practice, to patients based on patient assessment and care planning procedures
- Perform nursing interventions such as taking vital signs, applying aseptic techniques including sterile dressing, ensuring infection control, monitoring nutritional intake and conducting specimen collection
- Administer medication and observe and document therapeutic effects
- Monitor patients’ progress, evaluate and document effectiveness of nursing interventions and collaborate with appropriate members of health care team
- Provide safety and health education to individuals and their families.
- Conduct initial and ongoing assessments of the patient’s physical, mental, and emotional health in accordance with established nursing care standards.
- Develop and implement a personalized care plan in collaboration with the Family physician and/or family members, ensuring it reflects the patient’s current condition and evolving needs.
- Regularly measure and document the patient’s vital signs, including blood pressure, blood glucose levels, heart rate, respiratory rate, temperature, and oxygen saturation.
- Observe, report, and record any changes in the patient’s condition or behavior, including signs of mood swings, depression, distress, infection, or complications.
- Safely administer prescribed medications (oral, injectable, topical, etc.) according to schedule and physician’s instructions.
- Monitor for potential side effects or adverse reactions of medications and report findings to the physician or appropriate healthcare provider.
- Maintain an accurate log of all medications administered.
- Support the patient with personal hygiene, including bathing, grooming, oral care, toileting, and dressing.
- Assist with mobility, repositioning, transfers and use of assistive devices to ensure safety and comfort.
- Monitor dietary intake and ensure the patient receives meals in accordance with their nutritional plan.
- Document nutritional and fluid consumption and report any concerns to the family members and/or family physician.
- Assist in collecting specimens such as urine, stool, sputum for laboratory testing as ordered.
- Perform basic diagnostic procedures such as glucose testing, urinalysis, or wound swab collection, ensuring proper documentation and handling when requisitioned by family physician.
- Educate the patient and their family members on managing chronic conditions, medications, dietary needs, and infection control practices.
- Provide emotional support and encouragement to the patient to promote mental well-being and reduce anxiety or depression.
- Ensure the home environment is safe and free of hazards to minimize the risk of falls or injuries.
- Be prepared to respond to medical emergencies and provide first aid or CPR if needed until professional help arrives.
- Maintain accurate and timely documentation of all care provided, patient observations, medication records, and care plan updates.
- Liaise with physicians, specialists, pharmacists, and other healthcare providers as necessary to coordinate patient care.
- Provide regular updates to family members and address any questions or concerns regarding the patient’s condition.
- Set up, operate, and maintain medical equipment such as mobility aids, blood glucose monitors, or blood pressure monitors as required.
- Instruct family members in the safe use of any essential medical equipment.
